ما هو؟
التهاب في نسيج الدماغ، غالباً فيروسي، وقد يكون مناعياً.
الأعراض
حمى، صداع، تشوش وتغير سلوك، تشنجات، وضعف بؤري.
العلاج والتعامل
أسيكلوفير وريدي فوراً عند الشك، ورعاية داعمة، وعلاج التشنجات.

التعريف
التهاب الدماغ (Encephalitis) التهاب في نسيج الدماغ نفسه، ويختلف عن التهاب السحايا الذي يصيب الأغشية، وقد يجتمعان.
الأسباب
- فيروس الهربس البسيط (الأشيع والأهم لأنه قابل للعلاج)
- فيروسات أخرى، منها المنقولة بالبعوض
- التهاب دماغ مناعي (أجسام مضادة)
الأعراض
- حمى وصداع
- تشوش وتغير في السلوك والشخصية
- تشنجات
- ضعف أو اضطراب كلام، وانخفاض الوعي
التشخيص
- الرنين المغناطيسي (الهربس يصيب الفص الصدغي غالباً)
- البزل القطني وفحص الفيروس جزيئياً
- تخطيط الدماغ
العلاج
- أسيكلوفير وريدي فوراً عند الشك دون انتظار النتائج
- علاج التشنجات وارتفاع ضغط الجمجمة
- علاج مناعي للأنواع المناعية
الرعاية التمريضية
- تقييمات عصبية متكررة، واحتياطات التشنج
- ترطيب جيد ومراقبة الكلى مع الأسيكلوفير
- سلامة المريض المشوش ودعم الأسرة
- التأهيل لاحقاً للذاكرة والسلوك
نقطة للاختبار
تأخير الأسيكلوفير في التهاب الدماغ بالهربس يرفع الوفيات والإعاقة، فيبدأ مع أول شك.

What is it? Inflammation of brain tissue, usually viral and sometimes autoimmune.
Symptoms: Fever, headache, confusion and behaviour change, seizures and focal deficits.
Management: Immediate IV aciclovir when suspected, supportive care and seizure control.
Definition
Encephalitis is inflammation of the brain tissue itself, unlike meningitis which affects the membranes, though both may coexist.
Causes
- Herpes simplex virus (most important because it is treatable)
- Other viruses, including mosquito-borne ones
- Autoimmune encephalitis (antibody-mediated)
Symptoms
- Fever and headache
- Confusion and changes in behaviour and personality
- Seizures
- Focal weakness or speech disturbance and reduced consciousness
Diagnosis
- MRI (HSV usually affects the temporal lobe)
- Lumbar puncture with viral PCR
- EEG
Treatment
- Immediate IV aciclovir when suspected, without waiting for results
- Treat seizures and raised ICP
- Immunotherapy for autoimmune types
Nursing care
- Frequent neuro observations and seizure precautions
- Good hydration and renal monitoring on aciclovir
- Safety for the confused client and family support
- Later rehabilitation for memory and behaviour
Exam point
Delaying aciclovir in HSV encephalitis raises death and disability, so it starts at first suspicion.
